EDUCATION /AND EXPERIENCE:

Education: Bachelor Degree in Medicine and surgery or the equivalent from an accredited institution.

Experience: Completion of one year experience (internship) in recognized hospitals or health centers. Language: Good command of oral and written English and Arabic.

Required Licenses/Registrations: Saudi Commission for Health Specialty and Ministry of Health Saudi License / ACLS & BLS

JOB SUMMARY :

Serves as a general practitioner to ensure that evidence based, proper, and prompt medical care is provided to patients and to oversee all patient safety related issues for all patients by visiting them in their homes.


DUTIES AND RESPONSIBILITIES:

1) Interviews and evaluates all newly visited patients during his/her duty hours, and takes a thorough accurate medical history, including detailed medication history, socioeconomic, psychological and functional history, and documents all relevant clinical information in the patient’s medical record, according to policies.

2) Notify the attending physician about the new patient, and discusses with him/her the patient’s clinical condition and any medical details that warrant further or additional management, clarify with him/her the needed work up and management plan.

3) Ensures that all requested work up is done and that all ordered medications are administered to patients on time in their homes.

4) Performs daily visits on patient homes and documents findings in the progress notes his/her findings along with assessment and plan of care.

5) Responds immediately to patient’s medical complaints, evaluates and manages the patient with assistance of the concerned specialist or consultant, and documents findings and plan of care in the progress notes.

6) Documents all the indications and clinical justifications for all requested advanced diagnostic tests (e.g. C.T. scan, MRI.U/S) and reports in the progress notes.

7) Endorses at the end of duty time in detail all patients to his/her colleague including the pending medical diagnostic and therapeutic issues. 

8) Coordinates between the patient and all disciplines of health care team involved in the care provision including nurses, consultants, clinical dietitians, physiotherapist, social workers, laboratory departments and Insurance company doctors, etc.

9) Fills accurately and properly all required forms in patient’s medical record as defined by the homecare policies.

10) Completes properly and accurately the discharge summaries of his patient’s, and discusses the discharge instruction sheet and home medications with the patients.

11) Attend all departmental meetings and participate actively in all departmental projects and activities.

12) Participates in all quality and patient safety activities in the FUTURE CARE.

13) Acts as on active member of the FUTURE CARE and shares in all efforts that facilitate the provision of safe, uneventful and smooth care to FUTURE CARE patients.

14) Comply with the homecare policies and procedures related to the staff conduct, including but not limited to punctuality, dress code policy, professional ethics, protecting patient and family rights and confidentiality agreement.

15) Reports to the attending of patients and/or chairpersons of medical departments any relevant unresolved medical patient related problems. 16) Participates in continuous medical education activities and earns at least 30 CME per year.
